Leading the way in technology, the CIS Virtual Care Center offers cardiac care to our patients 24 hours a day, 7 days a week.

The VCC team is comprised of nurse practitioners, registered nurses, licensed practical nurses, telemetry technicians and care navigators – all specially-trained to provide the highest-quality cardiovascular care remotely.

Our VCC offers the following services:

  • Refills prescriptions
  • Triages patients by phone or video visit
  • Process prior authorization and pre-op cardiac risk assessments
  • Schedules appointments
  • Coordinates follow-up care

CardioConnect

CIS CardioConnect Patient Portal and Mobile App allow patients convenient, secure access to their health record, including lab results and the ability to request medication refills and appointments. To sign up, ask a receptionist for your special 8-digit passkey at your next visit. Click here to access the patient portal. The CIS CardioConnect mobile app can be downloaded from the Apple App Store. It is currently only available on Apple mobile devices.
